Is Kovu Scar's son?
It is clearly stated several times during the movie that Kovu is not Scar's son, despite the major resemblance. In the original script of The Lion King 2 Kovu was going to be Scar's son but it then changed because of the fact he would be falling in love with his close relative, Kiara.

Are Nuka and Vitani Scar's children?
It's never stated in the film if Nuka and Vitani are Scar's children. Infact, it's never even stated that Zira is Scar's mate. She is stated in books as "follower" not a "mate". Even so there are many quotes that shows Zira's love for Scar (ie. "And I, his loving, devoted queen") but there is no evidence that Scar loved her back. Zira may have only been a "fangirl" to Scar which may be the reason why she is so determined to have her own son follow his pawprints.

Are Vitani and Kovu twins?
Most probably. Although "twins" is perhaps not the correct word, "littermates" would be more suited.

Why is Simba's cub a girl/What happened to Kopa?
Simba's cub was probably changed from male to female for the plot to work out. As far as The Lion King 2 is concerned, Kopa never existed. There are many fanfics/rumours by TLK fans that Kopa was murdered by Zira which would explain why Simba is so protective of Kiara.

Where did Zira come from?
Zira was originally a member of Scar's pride. It's uncertain whether she was around before Scar was king.

Where did the Outlanders come from?
Like Zira, the Outlanders were also former members of Scar's pride.

When was Kovu born?
As stated by Zira, Kovu was the last cub born before Simba exiled Scar's followers to the Outlands. It is also suggested that Kovu was born during Scar's reign because he was hand-chosen by Scar. In which case Kovu is quite abit older than Kiara. Why he doesn't look any older I don't know.

Who was Kovu's father?
Kovu's father is unknown. Chances are he was just a rogue.

Where were the Hyenas in TLK2?
I always thought that the hyenas actually died at the end of The Lion King. Not many other people share this opinion. In The Lion King 2 Nuka mentions that the hyenas ran off.
